CENTRAL ASSESSMENT UNIT
Purpose of the Central Assessment Unit is to continue to be a single point of entry to:
1. Help Greene County residents maintain their maximum level of independence, self determination and dignity.
2. To reduce fragmentation and facilitate transfers between funding sources and levels of care.
3. Develop and maintain data of Long Term Care needs.
4. Use available resources efficiently and effectively.
The Central Assessment Unit works to provide quality services to citizens of Greene County regardless of age or income. If there is a health care need, the unit schedules free home needs assessments to evaluate the need for services and then makes a referral to proper agencies.
The unit determines, through the referral and the home assessment, the need for services of clients for any program it offers.
PCA (Personal Care Aide) - MA
LTHHCP (Long Term Home Health Care Program) - MA, Private Pay
CAH (Care at Home) - MA
ALP (Assisted Living Program) - MA, Private Pay
SCP (Senior Companion Program) - volunteer station, 29 SCV
CDP (Consumer Directed Program) - MA
PDN (Private Duty Nursing) - MA
TASA (Teenage Services Act)
PRI & Screen - As of January 1, 1996 , there will be a $50.00 fee for PRI and Screens. Medicaid recipients - waivered fee.
